Molecular biology of the placenta is a fascinating research subject to investigate the mechanisms underlying fetal growth abnormalities and developmental disorders. Analysis of Placental Gene Expression Profile in Abnormal Fetal Growth provides a new insight into the gene dysregulation and pathway changes in the placenta of pregnancies affected by fetal growth restriction and macrosomia. This book reviews the physiology of the placenta and its divers roles in maintaining a healthy fetal development. There is a summary of the aetiology of abnormal fetal growth and its adverse effects on the future of infants. The main focus of this book is presenting the findings achieved by microarray gene expression analysis in the placentas collected from growth restricted and macrosomic newborns, introducing some candidate genes for screening purposes. In addition, molecular function of dysregulated genes and affected biological pathways have been described in detail.

Placental Adhesive Disorders

Abnormal placental adhesive disorders are associated to massive hemorrhage and high maternal morbidity and mortality. The main risk factor for abnormal invasive placentation is the repeated cesarean, although other factors were identified. There are specific techniques to provide a high confidence diagnosis. However, precise skills must be acquired to recognize detailed diagnostic signs, to avoid common technical mistakes, and also to know when, how and why it is necessary to use each of them. Presurgical study provides diagnosis, extension and compromise of neighboring structures such as the bladder or the parametrium. Knowledge of placental invasion extension is needed to plan any resective surgery such as hysterectomy or one-step conservative surgery. Due to the fact that topography of the invaded area has direct relation with the specific arterial pedicles, a map of the invasion is required to know which type of proximal vascular control can be more effective. Leaving the placenta in situ seems to be the best option when resources or a skilled team are not available, but it requires intensive postoperative controls to detect infection, bleeding or coagulation disorders. Hysterectomy can be an easy solution for non-experimented operators, however, it is usually a very complicated procedure with demonstrated morbidity and mortality due to hemodynamic and hemostatic problems. This book gathers the latest knowledge in relation with the etiology, diagnosis, treatment and also the authors personal experience in more than 500 cases. All aspects of this condition have been analyzed to provide an accurate management, which includes vascular control, urology, anesthesia and hemodynamic management among others.

The Genetics of Familial Hydatidiform Moles

Hydatidiform Moles are an abnormal pregnancy in which no foetus forms. Instead, the placental villi proliferate as fluid filled sacs with tumourigenic potential. This is a relatively common disorder occurring in 1 in 1500 pregnancies in North America, and even higher in other populations. In certain women hydatidiform moles have been found to recur, and in even rarer cases they recur in multiple women in the same family. These recurrent cases have allowed identification of a genetic defect causing these moles.Mutations in NLPR7, a gene implicated in the innate immune response, were originally identified in four families with recurrent moles, pinpointing it as the gene responsible for this disorder. This work introduces the history of molar research, describes the identification of the causative gene and gives insights into possible mechanisms for the manifestation of this disorder.

Major Degree Placenta Praevia

Antepartum haemorrhage is a significant aetiology of maternal and fetal morbidity and mortality. One of the most important causes is placenta praevia and is increasing with the rising incidence of caesarean section operations together with advanced maternal age. The incidence of abnormal placental invasion has increased 10-fold over the past 50 years. Placenta praevia exists when the placenta is inserted in the lower segment of the uterus. Morbidly adherent placenta praevia (MAPP) could pose as a life threatening obstetrical complication and is distinguishable by the attachment of the villi to the myometrium without penetrating the thickness of the muscle. The aim of this study was to examine maternal and fetal clinical outcomes in association with morbidly adherent placenta praevia and assess the reliability of various diagnostic modalities available at a tertiary medical facility for optimal management of patients with major degree placenta praevia and MAPP.

Human Fetal Growth and Development

This unique book delves into the mysteries of human fetal growth and maturation. Growing knowledge in genetics indicates that factors that impact on/influence fetal growth and maturation may have a role in determining a person's health and disease in later years. Placental, maternal, environmental, nutrient as well as fetal genome factors each play a role in producing a healthy, unhealthy or abnormal baby. A study of fetal growth and maturation is therefore basic to the understanding of why fetal growth problems occur, what implications these can have for adult disease, and how clinical intervention can help to reverse growth problems. The present study will be comprehensive and will be a major contribution to the fields of gynecology, genetics, obstetrics, biochemistry, molecular biology and clinical medicine. It will include cutting edge research in the field as well as explorations on clinical interventions in fetal growth, which will not only add to existing knowledge but also prompt future research. The two Editors are distinguished in their fields and both have extensive clinical and research experience. They felt that they could use their expertise to create a book that will help students, practitioners, researchers and others to understand the subject of gestation, growth and maturation and its implications from a multi-dimensional point of view, which will help them develop their own expertise in a cutting-edge and developing field. They have brought together medical scientists, clinical practitioners, embryologists, endocrinologists, immunologists, gynecologists, obstetricians, reproductive and molecular biologists, geneticists and many others to create a state-of-the-art book on a subject with increasing demand for further knowledge. It aims to integrates different disciplines to give a holistic view of human fetal growth maturation.
